I'm not really sure if this constitutes as a pre-production question, but whatever. Does anybody happen to know where I might be able to get a hold of a sample release form? I am interested in shooting some things, but would like to make sure I'm doing everything "by the book" (no, not THAT book, you Kevin Smith freaks).

nice find. i really like his article on reasons for using original music. it brings home the point pretty well.
